<p>I am now re-reading this book. The writing is very magaziney and shallow. The descriptions of Cabllo Blanco and Jenn Shelton for example are intensely overwrought. However, the topic is fascinating, the insights unique, and information fantastic. For example, yesterday I made a pair of huraches and will go trail running in them today. (If you want info on how to make a pair, drop me a line.) This is a book that motivates action, causes me to rethink the basics of running, and is worth reading twice for the info.</p>

			<content:encoded><![CDATA[<p>The paperback of <em>Chasing Medical Miracles</em> is due out on May 25. There is a newly written foreword about swine flu and the clinical trials process for that. I will excerpt that foreword on this site this week and in the days leading up to the release of the paperback.</p>
<p>If anyone is looking to buy the book for a class or symposium &#8211; or, of course, for themselves &#8211; the cost will be significantly less than the hardback version, narturally.</p>

			<content:encoded><![CDATA[<p>When I travelled to Uganda as part of my research into my book I wanted to stop into a clinic in Kampala that was run by Pfizer. I contacted their media office and we went around and around and was told I couldn&#8217;t visit the Infectious Disease Institute (IDI), which is on the grounds of Mulago Hospital. The reason was that only a limited number of trials were conducted there. Once in Kampala and interviewing officials I asked about the Pfizer clinic. One prominent doctor at Nsambya Hospital said IDI conducted a significant number of trials. A Ugandan health official said no trials were conducted at IDI. I was left wondering what was accurate: some trials, no trials, or lots of trials.  What I was sure of was that Pfizer appeared to be a slippery operation when it came to trials . 			<content:encoded><![CDATA[<p>This weekend I am gong to go out to the cineplex and see what has been described as a not-so-great movie called <em>Extraordinary Measures. </em>The story about a businessman (played by Brendan Fraser) who teams with an irascible scientist (apparently overplayed by Harrison Ford)  to develop a drug to cure a disease that fatally affects a small number of children is, according to what I have read so far, notable for insights it provides into the world of clinical trials. The <em><a href="http://www.thestar.com/">Toronto Daily Star</a> </em>dissed the film but did concede it was a &#8220;&#8230;movie that actually works better as an instructional video on the perils of the American medical system than it does as straight melodrama. Few films on this topic so clearly illustrate the competing agendas and cut-throat capitalism of America&#8217;s for-profit health care.&#8221; (For full review click <a href="http://www.thestar.com/entertainment/movies/article/754210--extraordinary-measures-life-and-death">here</a>.)</p>
<p>The film is notable because of how the worlds of commerce and business are so closely coupled with medical research. There is a creeping acknowledgement that research is not pristine, nor purely about the discovery of medical breakthroughs; that it has a commercial side that can be viewed as less than savory. This is similar to how medicine overall  has been increasingly portrayed as a corporate enterprise more than as a life-saving endeavor.  Whether the movie&#8217;s views about research and money are wholly accurate is beside the point. It does signal that the clinical trials system is facing an image issue &#8211; one that  CROs as an industry and university researchers as a group might want to be aware of as the business of research continues to grow at an impressive rate while other sectors of the economy tank.</p>
<p>For  more reviews of Extraordinary Measures go to <a href="http://www.rottentomatoes.com/m/extraordinary_measures/#">Rotten Tomatoes</a>, which links to several reviews and provides a good oversight of the film &#8211; which has been compared to a glorified made for TV movie. But hey, those of us interested in trials will certainly get something out of the experience, if only a little more insight into how the general public views our endeavors.</p>

Now comes the glass half full part: The editor in chief of the Journal of the American Medical Association says that before the request for registration the number of trials that would have registered would have been “zero.” The editor sites the study results as real progress in getting compliance and full disclosure in trials results. There’s no word on what if any steps will be taken to ensure a higher or increased rate of compliance in the future… in other news, the swine flu vaccine keeps marching on and the number of trial in India grows by huge rates…]]></description>
